5 Easy Steps to Get Started
Step 1. Call Our Office
To start the process, please call our office at (770) 443-9672. One of our Client Coordinators will complete what is referred to as an “initial intake”. This intake is what gets your “foot in the door” as it is how our office is able to determine the necessary documents needed for your child, obtain all contact information, and collect insurance policy information.
Step 2. Verification Of Insurance Benefits
We call you to explain your therapy benefits outlined by your insurance company, including coverage limits or exclusions, as well as any deductibles, co-pays, and/or co-insurance you are responsible for.
At this time, you will be able to confirm whether or not you would like to pursue an evaluation and/or treatment with our facility. To check if we accept your insurance, please refer to the list below
Step 3. Obtain and Send in Required Documents
Following the initial intake, we will send you a follow-up email with the list of required documents we will need prior to scheduling an appointment. These include but are not limited to a therapy referral from your child’s pediatrician, a current hearing test, current IEP/IFSP, current therapy evaluations and/or Plans of Care. All required documents can be faxed to our office at (770) 505-3595 or scanned and emailed to mckennafarms@gmail.com. Contact our office if your doctor requests a referral / prescription request from our facility.
Step 4. Schedule
Once we have received all required documents and we are able to offer your child a therapy spot, we will contact you to schedule your child’s first appointment. Please note that we will offer you a therapy time slot that is consistent week to week.
Step 5. Complete your Client Registration Packet

We request that the Registration Packet be completed at least 24 hours prior to the start of your child’s first scheduled appointment. This gives our therapists time to thoroughly review your child’s information to best prepare for the initial evaluation or first treatment session. Please note this packet is required to be renewed annually.
